The short answer

Use contextual targeting when the environment or current content is the useful signal and audience targeting when a documented, permitted person or household signal materially improves selection. Many DTC plans need both, separated into measurable cells so their overlap and contribution are visible.

Understand what each method claims

Contextual targeting selects pages, videos, apps, or content categories. It does not establish the viewer's diagnosis. Audience targeting selects an addressable identifier based on a segment rule; it does not guarantee the person currently has intent or will see the ad in a suitable context.

Evaluate context beyond keywords

A health term can appear in news, professional education, misinformation, satire, or a personal story. Review page-level semantics, publisher quality, language, sentiment where defensible, and adjacency. Suitability is brand-specific; avoid calling all sensitive health content unsafe by default.

Evaluate audience evidence

Apply the provenance, construction, refresh, identity, and validation checks described in the DTC audience due-diligence process. Verify addressable scale inside the buying platform and compare the matched and unmatched populations.

Design a hybrid test

Create mutually understandable cells: contextual only, audience only, overlap, and a broader eligible baseline if feasible. Align geography, creative, frequency, timing, and outcome definitions. Report delivery quality and business outcomes separately.

Hypothetical reading

Suppose contextual media reaches more unique households at a lower cost, while the audience cell shows a higher attributed conversion rate. Without a counterfactual, that difference may reflect selection rather than media impact. The investment decision should consider incremental reach, causal evidence, and marginal cost, not declare a universal winner.

When advice changes

For rare conditions, audience scale may be fragile and contextual education valuable. For broad categories, contextual reach can be abundant but require strict suitability controls. For suppression or message sequencing, identity-linked approaches may be operationally necessary. Promotional requirements apply to the creative regardless of the targeting method.

Practical takeaway
