The HCP Journey: Stages From First Awareness to Advocacy
The HCP journey explained: the stages a prescriber moves through with a new therapy, what each stage needs, and which channels and signals fit each one.
The short answer
The HCP journey is the path a healthcare professional follows with a therapy, usually from awareness, to understanding, to first prescription, to routine use, and sometimes to advocacy. Each stage needs different information and fits different channels. Mapping it helps brands give each channel one clear job and choose signals that show real movement rather than activity.
Brand teams talk about moving HCPs "down the funnel." The HCP journey is a more honest version of that idea. It describes what a clinician needs at each point before a therapy becomes a normal part of their practice.
The five common stages
| Stage | What the HCP is thinking | What helps |
|---|---|---|
| Aware | "I have heard of it." | Broad reach, simple core message |
| Informed | "I know who it is for and how it compares." | Clinical data, dosing, safety, peer perspective |
| Trialing | "I will try it with the right patient." | Patient selection support, access help, samples where allowed |
| Adopting | "It is part of how I treat this condition." | Reminders, support programs, new indications |
| Advocating | "I recommend it to colleagues." | Peer programs, data updates, speaking opportunities |
Before launch, many brands add a disease-education stage. Promotional messages cannot run before approval, but unbranded education about the condition can. See disease-state insights for pre-launch planning.
Why a journey beats a funnel
A funnel implies everyone moves in one direction at one speed. Real HCPs do not:
- An oncologist may adopt a therapy for one tumor type and stay unaware of a second indication.
- A safety label change can push an adopter back to "informed."
- A high-volume prescriber may skip trial and adopt quickly because a respected colleague did.
Mapping by stage lets you choose the next useful message for each HCP, not the next scheduled message.
Channels by stage
- Awareness: NPI-matched programmatic, endemic display, CTV for HCPs where scale exists. See CTV for HCPs.
- Informed: email, medical education, endemic content, field discussions.
- Trialing: field, point of care, access support content.
- Adopting: point of care and EHR reminders, email updates, support program promotion.
- Advocating: peer-to-peer programs and medical meetings, handled within company policy.
The article on sequencing HCP media explains how to use observed signals to move an HCP from one channel's job to the next.
How to tell which stage an HCP is in
You rarely know for sure. You infer it from signals:
- Exposure and reach suggest awareness is possible, not certain.
- Engagement with clinical content suggests the HCP is gathering information.
- First prescription in claims data marks trial.
- Repeat prescriptions across patients suggest adoption.
- Speaking, referrals, or survey responses may suggest advocacy.
Each signal has error. The piece on measuring engagement at each journey step covers which signals to trust.
Building a simple journey map
- List the stages that fit your product.
- For each stage, write the one question the HCP needs answered.
- Assign approved content that answers it.
- Assign the channels that can deliver that content.
- Choose one observable signal that suggests the HCP has moved on.
- Decide what happens when the signal appears, and what happens when it does not.
Keep it to one page. Journey maps that run to thirty slides are rarely used after the workshop.
Common mistakes
- Defining stages by marketing activity ("received three emails") instead of HCP behavior.
- Treating clicks as proof of movement.
- Ignoring that different specialties may be at different stages.
- Building a map with no data to place HCPs on it.
Practical takeaway
Pick the stage where most of your target list is stuck today. Put most of the next quarter's effort into the content and channels that answer that stage's question, and track one signal that shows HCPs moving past it.
Frequently asked questions
What are the stages of the HCP journey?
A common version has five stages: aware, informed, trialing, adopting, and advocating. Some brands add a pre-awareness stage for disease education before launch.
Is the HCP journey linear?
No. HCPs skip stages, move backward after a bad experience or a safety update, and can sit at different stages for different patient types.
How is the HCP journey different from the patient journey?
The HCP journey tracks a clinician's adoption of a therapy across many patients. The patient journey tracks one person's path from symptoms to diagnosis and treatment.
