Direct, PMP, or Open Auction for Pharma Media?
Compare direct, private marketplace, and open-auction buying for pharma across control, scale, transparency, cost, and measurement.
The short answer
Use direct buying when guaranteed placement or a specific publisher relationship is essential, a private marketplace when selected supply and deal-level controls matter, and the open auction when flexible scale and discovery justify stronger filtering. No route is inherently high quality. Seller authorization, page context, app identity, auction mechanics, and actual delivery still require validation.

Start with the media job
A medical-journal homepage sponsorship and a broad DTC contextual reach layer solve different problems. Define whether the plan needs a guaranteed moment, a trusted set of environments, unique data, incremental reach, or price discovery. Then choose the transaction.
For branded prescription-drug creative, promotional review and balanced benefit-risk presentation remain relevant regardless of buying route (FDA). A curated deal does not transfer that responsibility to the publisher.
Audit the supply path
For programmatic deals, verify:
- the publisher or app being represented;
- whether the seller is direct or an intermediary;
- the authorized seller relationship in ads.txt or app-ads.txt;
- the seller identity in sellers.json;
- the number of hops and duplicate paths;
- the actual domains, apps, formats, and geographies delivered;
- viewability, invalid traffic, suitability, and frequency after launch.
IAB Tech Lab defines ads.txt as a mechanism for publishers to declare authorized digital sellers and sellers.json as a way to identify entities selling or reselling inventory. They improve transparency but do not certify media quality (ads.txt; sellers.json).
A practical portfolio approach
Use direct placements for scarce, high-value environments; PMPs for repeatable access with negotiated controls; and open auction supply only where allowlists, exclusions, verification, and log-level review are workable. Test overlap because the same impression opportunity may appear through several paths.
Hypothetical decision
A DTC video plan sees strong completion in a PMP but limited incremental household reach. Open-auction expansion finds new reach, yet quality variance rises. The correct response may be a capped open-auction layer with stricter supply controls, not replacing the PMP or declaring one route universally superior.
Practical takeaway
Choose buying mechanics after defining the job and evidence standard. The next step is a route-level scorecard that compares unique reach, total cost, supply-path transparency, quality, and measurable outcomes rather than CPM alone.
